Apply for the health card in Córdoba

Prepare your application for the health card in Córdoba: your identity, your address and the document that proves your right to healthcare.

The health card identifies you as a patient and helps you book appointments and other services. If your details are right, they will not open a duplicate record or write to an old address.

The essentials

What it costs
The official information does not mention any fee to ask for the card. If you pay for a private help service, that is separate from the healthcare procedure.
How long it takes
We cannot give you a delivery date. Registration, recognition of your right and the physical card may come at different times. Ask which receipt you can use while you wait.

Is it for you?

People who live in Córdoba, who already know how they are entitled to healthcare, and who need their health card or document.

  • Do you already have a health number or card from this region?
  • Is this the first time you ask for it, or do you need another one?
  • Are you asking for yourself or on behalf of someone else?

What to bring

Tick each document when you have it ready. Ticks are not saved.

  • Which one works in your case

Look for your previous card, or ask the health centre to check whether you are already registered before opening a new record.

Step by step

  1. Check what you need: your first card, a transfer, a reactivation or replacing the one you have with a new one.
  2. Ask for the Andalusian health card at your health centre. When you are registered, you are also assigned a centre and a family doctor or paediatrician.
  3. Before you apply, check your name, your address and your phone number.
  4. Keep the receipt and ask how you will get the card: whether they send it or you collect it.

Afterwards

  • Check that the details on your card or document are correct.
  • Confirm which professional you have been assigned and book an appointment through the official channel.

Common mistakes

  • Asking for a new registration when you are already registered.
  • Confusing asking for a new card with having your right to healthcare recognised.
  • Not updating your address or phone number.

What this guide does not decide

Being registered and having the card do not, on their own, decide who pays for each medical service. Follow the instructions for your type of cover.
